All placements through Positive Pathways Solutions will be planned, purposeful and positive. All members of the care team aim to meet individual needs of all children in our care.
All children will have individual risk assessments, unique to them and their needs. They will also be assinged a key worker upon admission and will also have weekly therapy sessions with a trained psyotherapist.
We have a robust admissons procedure offered on the outcome of the completion of a referral risk impact assessment, the needs of the child against the provision of the home and with the other children in the placement.
Positive Pathways Solutions strives to give all children in our care a place to call their own. We want all our children to stay with us for as long as they need.
This can be until they are ready to go back into the family home, move into a foster placement or special guardianship, or go into semi/independent living.
While in the home, all children will have a education plan to ensure that they meet their goals.
As well as this, all children will take part in our Life Skills Programme. Here they can gain knowledge and understanding of factors or challenges that they may come up against when they leave home.
They will also learn new skills such as cooking and budgeting.
All children in the home will take part in our Life Skills Programme. The programme assists children in gaining knowledge and skills ready for when they leave care. This programme will also contribute towards their Pathway Plan when they turn 16 years old.
All children on the pathway plan will be supported to complete it with help from the care team. All children will also have education and a transition plan which will document what their next steps are.
